For TeleHealth appointment, please log in here and follow instructions below

1. Enter your Self-Service ID and Password.

2. Confirm Birthdate.

3. Click 'Profile' on left-hand panel and update required information.

4. Click 'APPOINTMENTS' on left-hand panel.

5. If prompted, click 'Complete Questionnaire.'

6. Once complete, click 'Appointment Check-in.'

7. Confirm Check-in is complete and surveys/questionnaires have been completed.

8. LOGOUT on top right corner.
