Club Forms
Please note that clubs/organizations must be ratified through the Inter-Club Council (ICC) to host official events, meetings, and workshops. Forms can be downloaded, electronically filled out, and emailed to the ICC advisor once completed. For questions, please call (714) 564-46217 or email clubs_orgs@sac.edu.

Please fill out this form to remain Ratified and active for the upcoming semester.
This form will be used for all club-related workshops and events. You can also request a meeting room for club meetings using this form.
Provide details of the proposed activity your club or organization is participating
in, or hosting. As well as the location of the event/meeting/activity dates the facilities
will be used. This form must be signed by the Club President, Advisor, and Student
Life.
Please email the completed form to the ICC Advisor at clubs_orgs@sac.edu. Additional forms may be needed depending on the event.
This form will be used when asking ICC for funding for club/organization events.
If your club/organization would like to request ICC funding, this form, the club budget balance (from the Student Business Office), the activity approval form (above), and flyers/invoices must be turned in 2 weeks before the event.
ICC will fund a proposal of up to 50% of the total cost of the program, event, or
conference for the club/organization. The amount of funding provided shall not exceed
25% of the remaining ICC budget balance allocated for funding club/organization activities. See
the ICC Constitution for more information.

If your club/organization is attending a conference, this form must be filled out
by your advisor and submitted to the ICC advisor three weeks before the conference. Along
with an activity approval form (above), schedule of the conference, student participation
waiver (below), conference registration, hotel quotes, and airfare quotes.

The check request form must be filled out with sufficient information that details the expense/purchase.
- The Requestor and Payee cannot be the same person
- All requests must be accompanied by backup documentation
- Administrator/Dean/Director approval is required on all check requests
- Club accounts must include meeting minutes and budgets with each request that shows approval of requested expenditures.
The Student Participation in District Sponsored Field Trip or Excursion Waiver form must be completed if traveling to a conference/event off campus.
The club/organization Advisor must fill this out with the roster of students.
Planning a fundraiser?
The Authorization form must be submitted prior to the fundraiser.
Revenue potential form is to be completed as a recap with 1) the estimated amount of funds before the event and 2) actual amount of funds after the event.
Fill this form out for limited engagement services provided by a guest speaker, artist, performer, workshop leader, trainer, lecturer, DJ, or education service provider for less than $15,000 and requiring a single payment after services rendered.
